Shemale (also she-male) is a term used (but not exclusively) in sex work to describe trans women with male genitalia and augmented female breasts from breast augmentation and/or use of hormones.

Therefore they are males pretending or wanting to be female but still male.

They are not pretending to be female, that's called cross-dressing, or drag.
