You're still a member, you don't need to make an Ex-member application. In regards to your 'Booster rank' You receive this when you're actively boosting the SmG discord server & it will be removed if you're no longer a booster. Booster on the forums isn't automatic, so you may have to wait for a while before your rank is displayed on your profile.
Feel free to join the discord server whenever and ping a staff member & ask for your roles.
