5. i got the game from my friend who copied it to me off his USB drive.
6. i have no idea, i just tried to enter a server and it said i was banned.
7. maybe this sounds selfish, but i honestly have no recollection of being banned from any server, and would like to be unbanned from smg 00 as soon as possible, i have never cheated, botted, or hacked in any way, and have never done anything to ruin other people's playing.

Most likely you are using a cracked cd key and someone with the same key was already banned.
You will need to get another key. PM Cloud, he may have one you can use but don't expect a quick response as he gets very busy at times.
